Voting form for Pulse diskmagazine

Fill out the sections of the form that you want to send it to us. Either
call us by modem (+358-0-5055041, Handle: PULSE, Password: EXTREME) or send
it by mail to:

To encourage voting and to give you some details  about  what  and  who  the
charts are based on, we will publish a  'List Of Voters'  each  month.  Here
we will publish the names of everybody who has voted  and  also  the  number
of voters. If, for some strange reason, you don't want your  name  published
but still want to vote, you have that possibility too.

RULES for voting
================

1. You may not vote on your own group's demos or intros, nor on your members
   or on the group itself.

2. You may not vote on  Extreme,  nor  on  our  demos,  intros  or  members.

3. You must fill out at least three of the entries of each section. If there
   are some sections you don't know anything about,  skip  them  altogether!

4. If you want your vote to be counted for issue 4, send it within June!

Votes that do not fulfill these rules are discarded! Observe that you should
write the entries in order from best to "worst".
